Sam and Dean go to investigate an outbreak of Swine Flu. Driving away from the town, Crowley shows up in the backseat of the Impala. He tells them he can get them Pestilence.
Crowley doesn’t know where Pestilence is, but he knows the demon that knows, and he tells Sam and Dean that if they can bring the demon ti him, he will get the information.
Crowley invites Dean but tells Sam that he isn’t invited. Dean decides to accept. Sam calls Bobby to complain. Then Sam suggests that he say yes to the devil and he jump into the cage. Bobby loses it on him.
Meanwhile, Crowley takes Dean to the pharmaceutical company that Pestilence operates out of and send him to the 12th floor on his own. When Dean gets up there, the demon says he isn’t interested in the rings from Death and Famine, and instead tries to kill Dean. Dean gets in the elevator, and when he gets out on the ground floor, the demon is about to kill him from behind when Crowley attacks the demon and bashes him in the head, knocking him unconscious.
On the drive back, Crowley says they can’t take the demon back to Sam because they have history. It turns out the demon is Brady, a college friend of Sam’s, and the person who introduced him to Jess. Brady tells Sam that Brady hasn’t been Brady since college. He tells Sam that when Brady changed after going on a semester break and came back differently, it was really him. Brady also confesses that he was the one who killed Jess.
Meanwhile, Sam has locked dean in the bathroom while he talked to Brady.
Crowley comes back and tells Brady that he has given the demons the impression that he and Crowley are lovers in league against Satan. As they are trying to get information on Pestilence from Brady, they hear hellhounds. Crowley reveals that demons have planted a tracking device on him. Crowley disappears and then shows up again with his own hellhound, allowing them to escape.
Crowley leaves and the brothers kill Brady. Crowley then shows up at Bobby’s and tells him he knows how to get Death. He tells Bobby he needs his soul to make a wish. He tells Bobby he’ll give it right back and then…credits.
