The episode opens with a young woman, Anna, in a hospital where she is being interviewed by a psychiatrist. She is telling the woman that she was trying to warn “them” that the apocalypse is coming. While she would sound crazy, her story matches what is going on, with Lilith trying to break 66 seals to bring forth Lucifer who will bring forth the apocalypse. Later, an orderly comes into Anna’s room to give her drugs, and she sees that he’s a demon even though nothing is visible. After she sees it, then his eyes turn black. He locks the door and clearly intends her harm, and Anna moves a filing cabinet into him, knocking him into the door. She does this with her mind. After he is unconscious, she escapes.
Sam and Dean are hustling pool (Sam is pretending to be drunk) when Ruby shows up with information. She tells them about Anna. Dean is not happy to see her, but Sam gets all the information, and the next scene they are headed for the hospital, but Dean still isn’t happy about it.
We find out through flashback that Sam tried to make a deal with a crossroads demon to trade places with Dean, but the demon wouldn’t do it.
Sam and Dean talk to Anna’s psychiatrist and look at her drawings, which reflect recent events. They realize Anna is seeing what is actually happening around them. They go to talk to her parents and find them both dead, killed by demons. Sam sees a picture of Anna in front of a drawing that matches the window of her church and they realize that’s where she has gone.
They find Anna in the church and she already knows who they are and all about them. It turns out she just hears everything the angels say. She doesn’t think the angels even know who she is.
Ruby suddenly shows up and tells them that there’s a high level demon on their way. They argue a bit and then the statue of the Virgin Mary starts bleeding from the eyes, and Ruby says it’s too late. Sam hides Anna in a closet. When the demon gets there, Sam tries to exorcise it with his powers, but he can’t.
The demon expects Dean to recognize him from Hell, and Dean realizes it’s Alastair. Ruby disappears with Anna, and Sam and Dean jump out the window to escape.
Back at their motel, Dean wants to understand why Sam trusts Ruby so much. Sam reveals that she saved his life. In flashback, we see that Ruby came back from Hell as another blonde, and was sent by Lilith to kill sam. Instead, Ruby kills the other demon with her. Sam throws her out of the car after asking her about the body she was in. We then see the body Ruby is currently in as a coma patient, flatlining. She shows up at Sam’s with the medical certificate showing the patient was brain dead. She then starts teaching Sam to use his psychic demon powers. Somehow, that leads to sex.
Dean is taken aback by Sam’s level of detail, but tells him to continue.
Sam tells him how he went after Lilith, who ended up not being there. It was a trap and Sam is almost killed but Ruby shows up and saves him. As Sam finishes his story, housekeeping knocks on the door. When they say they don’t need housekeeping, the woman is pretty insistent. It turns out to be Ruby, who tells them where to find Anna. They go and find Anna, and right after they get there, Castiel and Uriel show up. They demand Anna. Sam asks if they’re going to help her and he tells he they aren’t, that Anna has to die.
Story & Character Analysis:
Dean is mad about Sam’s relationship with Ruby and wants to know what happened while he was dead. Sam is mad that Dean won’t tell him what happened to him while he was in Hell. When he does tell him, we see how far down the path of evil Sam has started to go.
We see more of the dichotomy between Sam and Dean and good and evil, as we find out that Sam has a sexual relationship with Ruby, a demon (the evil).
Sam’s bond with his brother is still quite apparent when he is about to go after Lilith in the flashback and we see that he wanted to die. He couldn’t live with himself after what happened to Dean, especially because he still sees it as his fault.
