The episode opens with a woman, Janet, brushing her teeth. While she is, a woman is chanting over a toothbrush. Then all of the woman’s teeth fall out and she dies.
Sam finds a hex bag under the sink in Janet’s house.
A woman, Amanda, comes home from the grocery store, and her neighbor mentions she missed book club. From her bandaged hand, we realize this is the witch. She opens her oven and takes out a chicken covered in flies and maggots. The camera cuts to Paul Dutton, eating a burger in his car. He puts it down for a second, and as she performs the spell, his burger becomes covered in maggots. As he starts choking, Sam and Dean pull up. Sam pulls a hex bag out of his car and sets it on fire, and Paul is okay. Dean demands to know who might want Paul and his wife dead, and he mentions an old affair. The camera cuts back to the witch and cuts suddenly appear in her arms, killing her so that it looks like she slit her wrists.
Sam and Dean find the witch’s body and think she killed herself and questions why she would do that. He then finds a hex bag under the witch’s coffee table. Sam surmises that they’re dealing with a coven…
…Which we then see is the “book club”.
The brothers approach Amanda’s neighbor and fellow “book club” member, Elizabeth. Sam tells her she must have a green thumb to get her herb garden to grow out of season. They find that the women from the “book club” have been having extraordinary luck, almost too extraordinary.
Sam says they have to stop them, and Dean questions if he means kill them. Dean points out that they’re human. Sam says they’re murderers and Dean looks troubled.
Ruby suddenly shows up in the road. She tells them that the witches serve a demon that is more than they can handle, and she is specifically talking about Sam. This leads to a major argument between Sam and Dean. Dean is completely against Sam working with Ruby. He brings up the fact that Sam is getting more and more okay with killing people. Dean confesses that he’s worried because Sam isn’t acting like himself.
Dean suddenly doubles over in pain and says it feels like he’s being stabbed with a bunch of knives. He realizes it must be the coven and Sam frantically starts looking for a hex bag, tearing the room apart. He has no luck and Dean has fallen onto the floor and starts coughing up blood. Sam then takes the Colt and leaves without telling Dean where he’s going. He shows up at the witch meeting with the Colt and tells them to let Dean go. They don’t appear to know what he’s talking about. Meanwhile, Ruby barges into the hotel room, picks Dean up off of the floor and pours blood into his mouth.
Sam surmises that one of the witches is the demon. He realizes all of them except Tammy have had newsworthy good fortune, so she must already be getting what she wants, the other women’s souls. Tammy drops the act, her eyes turning black. Sam fires the Colt but Tammy stops the bullet and it drops to the floor.
Tammy tells Sam that there’s a new leader, and the new demon doesn’t like Sam because he doesn’t want the competition. She’s about to kill Sam when Dean bursts in. Tammy throws him against the wall as well. Then Ruby comes in and tells Tammy that she’s missed her. Then she tries to stab her, but Tammy catches her arm. They then have a huge fight. It turns out that Ruby was a witch and sold her soul to Tammy, and that’s how she became a demon. Tammy starts to exorcise Ruby, but Elizabeth has gone back to the altar and started doing a spell on Tammy, which stops her momentarily. She then kills Elizabeth, but it was a long enough pause that her hold on Sam and Dean broke, enabling Dean to get the dagger Ruby had and go stab happy on Tammy.
Ruby shows up at their motel to talk to Dean. She confirms that all demons used to be human. He also confirms that there is no way to save him from going to hell. Ruby tells him he will lose his humanity as well. She also tells Dean that he needs to help her get Sam ready to fight the war on his own. Dean asks her why she wants the good guys to win. She says she isn’t like other demons. She remembers what it’s like being human.
After they find the witch’s body, Dean is startled by a dead rabbit hanging up.
Janet’s teeth all start falling out. Really gross.
